This Lifetime Achievement Award is named in honor of Rose M. Tate, former VASHRM member, in recognition for her notable dedication to VASHRM and her many outstanding contributions to the risk management profession. The first presentation of this award was made posthumously to Rose Tate’s family/employer at the 2008 Spring VASHRM meeting.
To recognize VASHRM members who have contributed to the advancement of the practices and principles of the Society through work in the healthcare risk management and patient safety professions, their community and other related organizations and have distinguished themselves through these endeavors.
1) Member of VASHRM for five (5) years or more
2) Active member on the Board of Directors of the Society for a minimum of four years
Any VASHRM member can nominate an individual to be considered for the award. All nomination forms must be typed and complete. Incomplete forms will be returned to the nominator.
Nominations for the Lifetime Achievement Award can be made by any VASHRM member with assistance from the nominee, as needed. Completed nomination forms shall be submitted to the VASHRM Membership Committee no later than the 1st of June of each year. The Membership Committee Chairperson, along with at least one other member of the committee and a past recipient of the award, shall review all nominations and present the top two candidates to the VASHRM Board for vote at the next Board meeting.
The nomination form for any nominee who is not selected for the award shall be held and considered as submission for the following year. Updates to the form will be requested prior to resubmission for further consideration.
Timing of the Award
The Award recipient shall be announced and presented with the award by the VASHRM President at the FALL meeting of the Society.
Type of Recognition
Award recipients shall be presented with a recognition plaque, a registration to a VASHRM Conference and a $100.00 gift card. Notice of the award recipient shall also be posted on the VASHRM website along with former award recipients. Information on the Award can also be made available to the recipients’ local newspaper by the VASHRM Secretary following approval and/or request by the recipient.
